Candace Perry is a Senior Manager, Solution Delivery at Arcadia, where she partners with provider and payer organizations to optimize clinical workflows, close quality gaps, and execute care management transitions with precision. Her work centers on the critical work of implementation and optimization: defining current state processes, identifying performance gaps and bottlenecks, designing workflows that work in practice, and building rollout strategies that minimize disruption while maximizing clinician adoption. She works closely with customers to establish key performance indicators that matter and develop training and change management plans that stick.
This hands-on expertise is grounded in 15 years at Tufts Medicine Integrated Network, where she guided 200+ primary care and specialty practices through the complexity of large-scale value-based care transformation. She spent that time in the trenches, understanding how practices operationalize change, where workflows break down, and what it takes to move populations through new models without losing momentum. She holds a Patient-Centered Medical Home (PCMH) certification and brings direct experience with the clinical, operational, and financial realities organizations face during transformation.
At Arcadia, working across both provider and payer organizations, she sees and helps clients navigate the tension points and dependencies that derail large-scale initiatives. She’s especially focused on helping organizations move from guesswork to structured, repeatable processes, whether that’s closing persistent quality gaps or migrating to new care management models without false starts.
Away from work, she’s a fixture on soccer fields across Massachusetts with her husband and two teenage sons, an avid true crime podcast listener, and never misses a chance to karaoke.
